We are a NHS GP Practice, based in Hounslow, West London. Our staff are committed to empower patients to change behaviour. We work with other health, social care providers as well as voluntary sector to help patients achieve this. We are exploring ways in which we can make smaller interventions to large number of patients with the use of technology to support healthy behaviour.
