Former preterm infants are not like babies born at full term. Having been born as early as 24 weeks these infants often go home many weeks before their due date weighing just over 4 pounds and often with multiple ongoing medical needs due to their prematurity. Until now these infants were sent home without additional resources to support their anxious parents who have become accustomed to having their infant monitored 24/7 and immediate access to medical assistance. Predictably, these infants have much higher ER visits and hospital readmissions compared to full term infants.
