Who We Are
Gilchrist is a nationally recognized, nonprofit leader in serious illness and end-of-life care. We provide counseling, support and care to people at every stage of serious illness, so they may live life to the fullest. Furthermore, we are deeply committed to giving people the clear information and loving support they need to make informed choices about their care.
A Trusted Leader
For more than 20 years, Gilchrist has been providing compassionate care and support for people with life-limiting illnesses. Most noteworthy, our programs offer comprehensive, coordinated care that addresses all of your family’s needs—whether they are medical, emotional, social or spiritual.
More than Hospice
You may know Gilchrist best for the care we provide people near the end of life. Yet, we offer so much more than hospice. Above all, today’s Gilchrist extends services to individuals with serious illness who need care and support well in advance of hospice care. While we provide hospice care for children and adults, we also provide counseling and support before and after the loss of a loved one.
Where We Can Serve You
Gilchrist provides care and support to families residing in Central Maryland, primarily in:
Baltimore City
Baltimore County
Harford County
Howard County
The following counties are served by our hospice program only:

Carroll County
Frederick County
Our Mission
To provide counseling, support and care to anyone with a serious illness, so they may live life to the fullest.
Our Vision
We are deeply committed to giving people the clear information and loving support they need to make informed choices about their care.

The Horizon Foundation is the largest independent health philanthropy in Maryland. We envision a Howard County free from systemic inequities, where all people can live abundant and healthy lives.
We work side-by-side with community to build power, advocate for policy change and dismantle structural racism to achieve better health for everyone in Howard County and greater Maryland.
Though our strategic priority areas and tactics have evolved over the years, our focus has always been to improve public health in our community, especially for those who face the most significant barriers to achieving good health. Since our founding in 1998, we have invested $78 million in public health, funding over 400 community groups and working with a number of partners on issues such as mental health, chronic disease prevention, health care access, healthy aging and more.
Our latest strategic plan demonstrates the Foundation’s bold commitment to lead as an anti-racist, trust-centered philanthropy, and to partner with community to drive impact in areas of greatest need. Our current focus areas are: demand an equitable health care system, increase affordability and build wealth and dismantle structural racism through community building.
